Request Form

Broward County apostille and notary services — Fort Lauderdale office by appointment, serving all of Broward County, filed in person with the state.

Request Form

What the form is.

Every apostille filing at the Florida Department of State is accompanied by the Apostille and Notarial Certificate Request Form — it tells the state who's requesting, what documents are enclosed, which country each is destined for, and where to send everything. The destination-country field matters more than it looks: it determines whether the state issues an apostille (Hague Convention countries) or a certificate of notarial authority (everywhere else).

You can download the official form below and complete it in advance — or skip it entirely. When we handle your filing, we complete the request form for you at your office appointment. It's part of the service, not an extra.

Request Form

Common form mistakes we see.

The three that cause problems: writing the wrong destination country (or leaving it blank), which can produce the wrong certificate type; listing documents that don't qualify, which stalls the whole envelope; and mismatched return information. Each is harmless when the filing is reviewed by someone who does this daily — which is the point of not doing it alone.

Filing Details

What the state does with the form.

The request form travels with your documents to Tallahassee and tells the Division of Corporations’ apostille desk three things: who is submitting, what is enclosed, and where each document is going. The destination country field drives which certificate gets issued — an apostille for Hague Convention members, or a certificate of notarial authority for everywhere else, which then needs embassy legalization on top.

If a shipment contains documents for more than one country, the form itemizes them separately so each receives the correct certificate. Get that wrong and the state issues the wrong instrument, which is not a rejection you find out about quickly — you find out when the receiving authority abroad refuses it.

You are welcome to download and complete the form in advance. When we handle the filing, we fill it out with you at handoff, confirm the destination for every document in the batch, and keep a copy on file so a reorder later takes one phone call instead of starting over.

FAQ

Common questions.

No — we complete it with you at your appointment. If you enjoy paperwork, the download is above; if you don't, that's what we're for.

Hague Convention countries receive an apostille; non-member countries receive a certificate of notarial/official authority that then goes through embassy legalization. The form's country field drives which one the state issues.

Documents bound for different countries are itemized by destination; we structure the filing so each document receives the correct certificate.
